At Prenato, we are committed to protecting the privacy and security of our customers’ personal information (hereinafter referred to as PI).
This Privacy Policy explains how we collect, store, use and protect your provided PI in accordance with applicable laws and regulations.

 

HOW WE COLLECT AND STORE YOUR PERSONAL INFORMATION

We use various methods to collect your PI: online forms, paper forms, e-mails, telephone discussions and consultations, face-to-face discussions and consultations or any other form of communication.

In order to have appropriate access to your PI, we use information and communication technologies to collect and store your PI, in compliance with the applicable laws in Quebec. We use a software that allows the use of personalized access codes to access your PI.

Despite all the protective measures we have put in place, no security mechanism can guarantee complete protection.

 

PERSONAL INFORMATION WE COLLECT

We collect your PI in order to provide you with medical services. This information may include, but is not limited to:

  • Identification information: Last name, first name, Quebec health insurance number, mailing address, e-mail address, telephone number
  • Financial and insurance information: credit card information, insurance company
  • Health information: weight, height, gender, age, medical information collection, medical results, medical forms, imaging

 

HOW WE USE YOUR INFORMATION

We use your PI to provide you with medical care or offer you various services:

  • Make and facilitate appointments
  • Provide you with the appointments and services you have requested
  • Maintain and update your health information
  • Contact you for medical follow-up, appointment reminders or satisfaction surveys
  • Manage billing and process payments

Your PI may be used, anonymously, to improve our services and for training purposes.

 

WHO HAS ACCESS TO PERSONAL INFORMATION

The following people may have access to your PI:

  • Employees of our company who need your PI as part of their duties and who have signed a customer PI confidentiality agreement.
  • Certain medical partners and service providers may have access to your PI for the purpose of medical follow-up or laboratory analysis. These partners are governed by applicable laws and by the code of ethics of their respective professional associations. Our company takes reasonable measures to ensure the protection of your personal information with these partners.
  • Depending on the services selected, some PI may be transmitted outside Quebec.

Prenato will not give, sell or rent your PI to third parties.

We will only transfer your PI with your consent, when required by law, or when there is a serious risk of death or serious injury.

 

HOW LONG WE KEEP YOUR PERSONAL INFORMATION

Your PI will be kept for a minimum of 7 years following the last service rendered by our company.

 

RIGHT TO ACCESS AND RECTIFY YOUR PERSONAL INFORMATION

You have the right to access, modify or delete your personal information. You may withdraw your consent to the collection and retention of your PI. Withdrawing your consent to the collection and retention of your PI may, however,
prevent our company from providing services to you. You may obtain a copy of your file by completing a medical record transfer authorization form. Fees may apply. Notwithstanding the above, our company may be required to retain some of your PI for the period prescribed by our
ethical obligations.
